Who is eligible to receive a headstone through this program?

Our program serves families of victims of a violent crime that took place in the Chicagoland area and who do not have a permanent headstone.  The deceased must be buried in a cemetery in the Chicagoland area.

Question 2

Who is NOT eligible to receive a headstone through this program?

Accepted Answer

A victim that provoked, incited, or participated in the criminal conduct that led to their death.

Question 3

What types of violent crimes qualify?

Accepted Answer

We serve families of victims of gun violence,  domestic  violence, and other forms of violent crime. If you are unsure whether your loved one's situation qualifies, please reach out to us — we are happy to discuss your circumstances.

Question 4

Can I apply on behalf of someone else's family?

Accepted Answer

No. You must be a parent, legal guardian, sibling, adult child, or an advocate acting on behalf of one of the previously mentioned family members provided they have the family's knowledge and consent.

Is there a cost to apply?

Accepted Answer

No. There is absolutely no cost to apply. Our program is funded entirely by donations, and we never charge families for the application or the headstone.

What information do I need to complete the application?

You will need basic information about the deceased (full name, date of birth, date of passing, city and state), the name and location of the cemetery, and your own contact information. Cemetery plot details such as section, lot, or block number are helpful but not required.

Can I submit a paper application instead of applying online?

Yes. A downloadable PDF application is available on our Apply page. You may print, complete, and mail or email it to us. Instructions are included on the form.

What happens after I submit my application?

Our team reviews every application personally. We will contact you to confirm receipt and may follow up with additional questions. The review process can take up to 4 weeks.  You will then be contacted for next steps.

How long does the process take?

Timelines vary depending on the volume of applications we are processing,  and the season/time of the year which has a direct impact on the headstone industry and cemetery management.  We ask for your patience and promise to keep you informed at every step.

Question 10

Will I be involved in choosing the headstone design?

Accepted Answer

Yes. We, in coordination with our headstone providor company, work with families to select an appropriate headstone.  While our program covers a standard headstone, we will discuss options with you and do our best to honor your wishes and your loved one's memory.

What if the cemetery requires specific headstone dimensions or materials?

We, in coordination with the headstone providercompany, will communicate directly with the cemetery to ensure the headstone meets their requirements. Please include the cemetery's contact information on your application so we can reach them if needed.

How are donations used?

Public donations go directly toward purchasing headstones and covering cemetery installation & foundation fees.  We do not charge administrative fees to families, and we work to keep our overhead as low as possible so that your gift has the greatest possible impact.

Is my donation tax-deductible?

Our organization is currently in the process of obtaining our IRS 501(c)(3) recognition. Our application is pending; tax-exempt status has not yet been granted. We will notify supporters once our status is confirmed.

What payment methods do you accept?

We accept bank to bank donations such as ACH payments and Zelle, checks, and all major credit and debit cards through our secure online donation page. All transactions are encrypted and processed securely.

Can I make a recurring donation?

Yes.  If you would like to give regularly, please contact us.  We are grateful for every contribution, no matter the size.

Can I donate to the memory of someone in particular?

Accepted Answer

Yes. When completing your donation, you may include a name of a person you wish for it to go towards in aiding in their memorilization through a headstone.
